Waivers

Build your own waiver, share its public link, handle minors and the medical questionnaire, and find signed copies.

Waivers (/admin/collections/waivers) are your shop’s own release forms. Each one has a public link at /waiver/<slug> that a participant fills in and signs on their phone, or on an iPad at the counter. Agency waivers that live inside SDI or PADI eLearning are not modelled here.

Build a waiver

  1. Create a waiver with a Title and a lowercase Slug. The slug is the public link.
  2. Set Trigger: Manual / shared link, On enrollment (training), On gear rental (POS) or On trip booking. Enrollment waivers are emailed automatically at checkout.
  3. Write a short Intro, then paste each legal section into Sections as a heading plus body.
  4. Under Fields to collect, tick Date of birth, Phone number, Emergency contact and Allow minors as needed.
  5. Optionally enable the Photo / media release, the Medical acknowledgement, and Include the full RSTC Diver Medical Questionnaire.
  6. For a rental waiver, enable the Equipment checklist and list the items.
  7. Check the E-signature consent line, tick Active, and Save.

What the participant sees

  • Their details, the sections, and any checkboxes you enabled.
  • A confirmation that they are 18 or older, or the box This participant is a minor (under 18) — a parent/guardian will sign, which asks for the guardian’s name, phone, email and date of birth.
  • A signature pad. They can draw, or switch to Type my name for a typed signature.

Signed copies

Each submission is rendered to a PDF and saved as a Signed Waiver, linked to the student by email. It appears under Signed Waivers on the contact profile with the waiver title, signed date and whether a physician evaluation is required. Opening one shows the participant details and the questionnaire answers.

To send a contact their waivers by hand, use Send waiver & medical on the contact profile header. It emails every active enrollment waiver, the same emails checkout sends.

CarefulWith the RSTC questionnaire enabled, a YES to a starred question or to any follow-up item puts the participant on the Physician Holds queue and blocks diving until a physician’s form is on file. See the Physician holds guide.
TipThe link in a Send waiver & medical email already carries the participant’s details, so it opens pre-filled — and the link itself is opaque, so it is safe to forward or text on. A bare waiver link from your site opens blank.
