Your waivers and training records: keep ours or upload your own

Every shop starts with DiveDesk’s waivers and the SDI/PADI referral forms. See each one, keep it, or put your own PDF in its place.

Settings → Waivers & Records is where a new shop decides what its students sign. Admin role only. Two sections: the waivers, and the training-record forms. Each card is either DiveDesk’s version or your own PDF, and you can switch either way at any time.

Waivers

Three arrive with your shop’s name already written in: a liability release with the RSTC medical questionnaire (signed at enrollment), a rental agreement with an equipment checklist, and a trip waiver with an emergency contact. Press Read the wording on any of them to see the full text.

  1. Happy with a waiver? Leave it. Edit wording & fields opens it for changes to the text, the fields collected, the questionnaire and the photo release.
  2. Have your own? Press Upload my own PDF instead and pick the file. From then on students read your document on the signing page, then fill in their details and sign beneath it. Date of birth, the minor/guardian option, the medical questionnaire and the other fields stay exactly as the waiver is configured.
  3. Need a waiver DiveDesk doesn’t ship? Use Add a waiver from your own PDF: a title, when it’s signed, and the file.
  4. Press These waivers are how I want them. That ticks the step off your setup checklist.

The signed copy a student receives is your PDF, page for page, followed by a page with their details, their answers and their signature — one document, filed under Signed Waivers on their contact profile like any other.

Training records

At the end of an Open Water course DiveDesk fills the agency’s referral form — SDI or PADI, whichever the course is — with the student, every pool and open-water date, the skills and the student’s and instructors’ signatures. Every other course (specialties, refreshers, advanced) gets a DiveDesk training record built the same way. Both cards show the blank form; Open the full form shows every page.

  1. Using the agency’s form as-is? Leave it.
  2. Your agency store issues a different record? Press Upload my own instead and pick the PDF. Your pages go first exactly as uploaded, then DiveDesk’s record page with the dates, skills and every signature. Instructors still sign in DiveDesk the same way.
  3. Press These training records are how I want them to tick the checklist step.
CarefulDiveDesk cannot write dates or signatures onto a PDF it doesn’t know, so an uploaded form is never filled in — it is attached in front of the DiveDesk record. If your agency requires its own form filled by hand, keep the built-in one, which is filled.
TipPDF only, 8MB, 30 pages at most. A password-protected PDF is refused; remove the password and upload again. Use the DiveDesk wording / Use the SDI form puts the built-in version back without deleting anything you uploaded before.
